All application, notification, and monitoring processes carried out by the Disabled Student Unit are conducted within a transparent, accessible, and recordable structure. Through this page, you can access the current forms and official documents prepared for students who wish to benefit from the services of the Disabled Student Unit.

Disability Notification and Report Renewal Form (2025–2026 Academic Year) Prepared for students with disabilities or special needs to report their current disability status and/or submit updated health reports. This form must be filled out in the following situations:

  • Students registering at the university for the first time who wish to declare their disability/special need status,

  • Students who have previously applied to the Disabled Student Unit and are renewing their reports,

  • Students whose disability status or health conditions have changed,

  • Students with temporary disabilities.

Things to Know Before Filling Out the Form

  • The information declared in the form is evaluated in accordance with the principles of confidentiality and the protection of personal data.

  • Shared health reports are used solely for evaluation and support planning purposes.

  • Academic advisors and instructors are informed only with the explicit consent of the student.

  • Filling out the form completely is important for the healthy execution of the support processes.
